When Making An Appointment:
If you are a new patient there are some basic forms that we will need you to complete. If you scheduled enough in advance of your appointment you will receive a packet from us with the required paperwork to be filled out at your leisure before you come in. If there was not enough time for us to get the forms to you we ask that you please come 15 minutes before your appointment time so that you can fill them out.
We ask that you please notify us of any special needs you may have that may require more time such as diabetes, glaucoma, cataracts, etc.... This will help us schedule the proper amount of time for your appointment.
It is your responsibility to know what your insurance benefits and coverage are. Insurance is not clear cut as in days of old and most all major insurances have dozens of different types of coverage so we can't stress enough how important it is to know exactly what your coverage is. We always try to help but we cannot be responsible for gathering your insurance information for you. There is a time limit involved in submitting claims and if, after submitting your claim it comes back as rejected by the time we get it re-submitted to the correct plan the processing time may have expired, in that case you would be billed directly. You cannot imagine how many people have us bill one insurance only to find out a month or two later that they have something else entirely!

On The Day Of Your Appointment:
If you are a new patient who received a packet of forms to be filled out please remember to bring your paperwork in with you.
If you take prescription medications please bring in a list of what you are taking so that we can include it in your folder. We will also need the dosages so please be sure to include that information.
All patients need to bring in your insurance card(s) which we will photocopy and keep in your file.
Please arrive in a timely fashion for your appointment. We do understand traffic but if you are more than 10 minutes late and we have someone due in after you, you will be asked to reschedule so as not to inconvenience the next person.
If you are a new patient we ask that you please bring in your current glasses and a box of your current contact lenses so that we can see what brand, base curve you have been wearing. This helps us to know where to start the process.
If you are a contact lens wearer you will be required to have them out before your exam begins so please make sure to bring a case with some solution (if applicable) with you. We do have basic cases available for a small fee in case you forget.
If you are a returning patient we ask that you please inform us of any changes you may have in your medications, address, phone number or e-mail address.
About Your Exam
When you come in for your appointment a comprehensive eye examination will be performed. This may include a number of different tests, such as visual acuity, refraction, ocular motility, ocular health assessment and pupil dilation.*
*To fully evaluate the retina, your pupils will need to be dilated with eye drops. The drops usually take between 15 to 20 minutes to be effective. After instillation, your vision will be blurred, especially when reading. In addition, your eyes will be more sensitive to bright light. These side effects are normally mild but can last a couple of hours. Please be sure to bring a pair of sunglasses with you to enhance your visual comfort after the dilation. Also, as a result of blurred vision, you may want to bring someone with you who can drive you home. If you are unable to be dilated at the time of your eye exam please let us know and we will try to make another appointment for you to come in for your dilation.
